How BMI is calculated

BMI (Body Mass Index) is calculated as weight in kilograms divided by height in meters squared:

BMI = weight (kg) / height (m)²

In imperial units: BMI = (weight in pounds × 703) / (height in inches)²

BMI was developed by Adolphe Quetelet in the 19th century as a population-level statistical tool, not a diagnostic instrument. Despite its widespread use, it has significant limitations at the individual level.

FDA GLP-1 BMI eligibility criteria

Per FDA approved labeling for Wegovy (semaglutide 2.4 mg) and Zepbound (tirzepatide), indications include:

  • BMI ≥ 30 kg/m² (obesity) — no comorbidity required
  • BMI ≥ 27 kg/m² with at least one weight-related comorbidity (e.g., hypertension, type 2 diabetes, dyslipidemia, obstructive sleep apnea)

Ozempic and Mounjaro are FDA-approved for type 2 diabetes and may produce weight loss as a secondary benefit. Prescribers have clinical discretion and may consider off-label use. BMI alone does not determine prescription decisions.

BMI limitations
  • Muscle mass: Athletes and muscular individuals may have a high BMI without excess fat.
  • Age: Older adults may have more body fat at a given BMI than younger adults.
  • Ethnicity: Asian populations tend to have higher metabolic risk at lower BMI thresholds (some guidelines use BMI ≥ 25 as the obesity threshold for Asian populations).
  • Fat distribution: BMI does not distinguish between visceral (abdominal) and subcutaneous fat, which have different metabolic risk profiles.
  • Sex: Women typically have more body fat than men at the same BMI.
